Who am I:
Tracey Duncan is a multi-disciplined therapist providing complementary therapies and natural beauty therapy, carried out in accordance with the code of practice and principles set out by the Complementary Health Professionals (CHP) association and the CNHC – Complementary & Natural Healthcare Council.

What information do I need:
I need your contact details, your name, phone number, address, postcode, and email address. I will also need information relating to your health, age and lifestyle habits in order to carry out treatments.

Why do I need this data:
I need this data in order to ensure that it is safe for me to provide the treatments that you have asked for and for my insurance to be valid. I will only collect what is relevant and necessary for your specific treatment. I will make notes which may include details concerning your medication, treatment and other issues affecting your health. This data is always held securely and is not shared with anyone not involved in your treatment. As part of my obligations as a healthcare practitioner there may be circumstances related to your treatment and/or on-going care that will require the sharing of your records with other healthcare practitioners e.g GPs, consultants, surgeons and/or medical insurance companies. Where this is required I will always inform you first and get your consent, unless I am under a legal obligation to comply.

How I will use it:
The lawful basis for processing this personal information is ‘contract’ this means that I have a legal reason to ask you for your data because I need it for contractual reasons. You do not have to share your personal data with me, but if you do not, then I cannot offer you a treatment with myself. You must fulfill your side of the contract (share your personal information) in order for me to fulfill mine (carry out treatments). Contact details provided by you such as telephone numbers, email addresses, postal addresses may be used to remind you of future appointments and provide reports or other information concerning your treatment.

For marketing purposes, I may also use the contact details provided by you to respond to your enquiries, including making telephone contact and emailing information to you which I believe may be of interest to you. You will be given the option at first face to face consultation to actively opt in or out of being contacted and the preferred methods of contact. Some basic personal data may be collected by filling out my website contact form or visiting my website such as: details of your visits to my website, including but not limited to, personally identifying information like Internet Protocol (IP) addresses. My WIX hosted website uses cookies, which is a string of information that a website stores on a visitor’s computer, and that the visitor’s browser provides to the website each time the visitor returns.
